Job Summary

As a Field Nurse Practitioner, you will provide essential screening, preventive primary care, and medical services to our members in comfortable settings such as their homes, communities, nursing facilities, and pop-up clinics. Your role will be pivotal in ensuring that our members make progress toward their healthcare goals, while also contributing to our mission of delivering quality and cost-effective care.

Essential Job Duties

  • Deliver comprehensive medical care and care coordination to diverse patient populations including adults, women, children, and the elderly.
  • Conduct thorough evaluations, including history and physical assessments, to identify care gaps and preventive needs.
  • Address both chronic and acute primary care concerns, while efficiently determining medical urgency.
  • Establish and document clear medical diagnoses; seek specialty consultations as necessary.
  • Order and perform relevant diagnostic tests judiciously, considering your resource-limited environment.
  • Create and implement individualized medical care plans.
  • Schedule and coordinate follow-up appointments when appropriate.
  • Minimize hospital readmissions and emergency room visits through effective post-discharge care coordination.
  • Conduct face-to-face visits in various settings, including in-home and skilled nursing facilities.
  • Utilize alternative modalities for visits as directed by leadership and in accordance with state regulations.
  • Participate in community-based pop-up clinics aimed at building relationships and addressing healthcare gaps.
  • Drive up to 120 miles daily, with potential extended travel for special projects requiring overnight stays.
  • Maintain cross-state licensure based on business needs.
  • Collaborate with other nurse practitioners to enhance best practices for efficient work.
  • Participate actively in regional meetings.
  • Prescribe medications and perform applicable procedures as permitted.
  • Ensure timely documentation in electronic medical record (EMR) systems.
  • Occasionally lift equipment weighing up to 50 lbs and navigate various settings, including staircases.
  • Engage in collaborative practices under the supervision of a licensed physician, adhering to state regulations.

Qualifications Required

  • Minimum of 1 year experience as a nurse practitioner or a similar educational and experiential combination.
  • Active national certification from AANP or ANCC.
  • Current, unrestricted Family Nurse Practitioner (FNP) license in your state of practice.
  • DEA license for prescribing in accordance with state regulations.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and reliable transportation with appropriate insurance for job-related travel.
  • Ability to adjust communication style working with diverse populations.
  • Self-motivated with minimal supervision.
  • Excellent problem-solving, time management, and communication skills.
  • Proficiency in Microsoft Office and EMR systems.

Preferred Qualifications

  • Experience in home health, community health, or public health settings.
  • Background in managing chronic conditions within home health.
  • Experience working with underserved populations facing healthcare barriers.
  • Skills in immunization and point-of-care testing.
  • Bilingual capabilities.
