Description
  • Conduct comprehensive evaluations of patients in either an inpatient or outpatient capacity and create customized treatment plans for each individual
  • Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
  • Collaborate with a multidisciplinary team including case managers, nursing staff, and physicians to ensure optimal patient outcomes
  • Provide education to patients and their families regarding therapeutic objectives, discharge processes, and at-home exercise regimens
  • Accurately document patient assessments, treatments, and progression in accordance with facility policies and regulatory standards
  • Engage in interdisciplinary team rounds and conferences to facilitate cohesive care for all patients
Education
  • A Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program
Certifications
  • Must be licensed as a Physical Therapist in the state
  • CPR certification is preferred
  • Additional certifications in specialized areas (e.g., geriatrics, neurology) are advantageous but not mandatory
Skills
  • Previous experience in a clinical setting strongly preferred
  • Expertise in treating patients with complex medical conditions and mobility challenges
  • Excellent communication skills and a strong ability to work as part of a team
  • Comfortable with digital charting and clinical documentation software
